Florida Care Connect Foundation does not provide medical services or clinical advice. We operate as a care coordination and referral organization.
Our work is sustained through community partnerships and licensed provider participation. Families never pay for access or referrals.

Travis T., a healthcare innovator and founder of ITM Enterprise, launched Florida Care Connect™ to solve the frustrating delays families and hospitals face when placing loved ones in care. With a background in automation and business growth, he saw an opportunity to bring speed, transparency, and compassion to Florida’s care ecosystem.
